Job summary

The Christ Hospital is seeking an experienced RN-Case Manager to oversee discharge planning and care coordination for acute bedded patients. You will review patient needs, coordinate with physicians, social work, and external agencies to ensure safe, efficient transitions.

Responsibilities include evaluating continued stay, planning discharge, and participating in rounds and hospital committees to improve outcomes and reduce delays. Ohio licensure and CCM/ACM credentials are preferred.

Qualifications

• Knowledge and application of a wide variety of advanced case management tools and methods.
• Knowledge of clinical and operations research methodology and design. Proficient in benchmarking and case management tools and techniques.
• Ability to operate PC based software programs or automated database management systems.
• Expertise in meeting regulatory and accreditation requirements.
• Strong presentation, written and oral communication skills with analytical and problem-solving skills and time/project management.
• Ability to work with a variety of disciplines and levels of staff across departments.

Responsibilities

• Clinical review of 100% acute bedded patients including Inpatient and Observation level of care.
• Monitors patients for appropriateness of continued stay, level of care and services, and quality of care using approved criteria.
• Discharge planning assessment, identification of needs, and social service intervention or referral.
• Awareness of services available to patients and families.
• Identify barriers to discharge and coordinate with external agencies.
• Communicate with physicians when alternatives to inpatient care are indicated.
• Develop discharge plans and educate patients and families.
• Facilitate daily huddles and multidisciplinary rounds.
• Participate in hospital committees as requested by management.
• Track avoidable day information in Midas per policy.
• Interface with registration, financial counselors, Social Work, and patient financial services to resolve financial issues.
