Qualifications

• Current state RN licensure and BLS certification required.
• 3 years RN experience required; care management exposure preferred.

Responsibilities

• Coordinate transitional care with interprofessional teams to ensure patient safety.
• Promote effective handoffs to the next level of care or community.
• Assess and mitigate risks during patient transitions.
• Develop individualized transitional care plans with patients and care partners.
• Participate in rounding, huddles, and committee activities.
