Advocate Health is seeking a Social Work Case Manager in Wake Forest Baptist Medical Center’s Nursing: Care Coordination department. The role is full-time with 40 hours per week and includes benefits eligibility.

The team handles discharge planning and psychosocial support, requiring CSWE-accredited MSW and licensure considerations. The position emphasizes patient advocacy, care coordination, and collaboration with a multidisciplinary team to drive safe transitions and optimize patient outcomes

Qualifications

• Identifies patients who would benefit from Case Management interventions based on discharge needs.
• Assesses data via interviews and evaluation of patient needs.
• Completes psychosocial assessments and identifies emotional and social needs.

Responsibilities

• Develops and implements psychosocial treatment plans, including crisis intervention and therapies.
• Initiates internal and external referrals based on patient needs.
• Collaborates with the multidisciplinary team to discharge plan progression.
• Maintains knowledge of payer and reimbursement practices impacting care plans.
• Documents in the EMR per department standards.
