Job summary

Advocate Health is seeking a part-time Case Manager in Winston-Salem, NC to identify patients who would benefit from care coordination and to develop a Plan of Care with the nursing team to ensure smooth transitions.

The candidate must be a graduate of an accredited nursing program with a BSN; MSN preferred, at least 5 years of clinical experience and current RN licensure in NC. This on-site role offers first-shift hours and a competitive benefits package.

Qualifications

• Graduate from an accredited school of nursing.
• BSN required; MSN preferred.
• Minimum 5 years of relevant clinical experience; Case Management experience preferred.

Responsibilities

• Identifies patients who would benefit from Case Management interventions based on discharge needs assessment.
• Interviews patient/family and evaluates data to determine needs.
• Develops and executes a Plan of Care with nursing to improve outcomes and cost efficiency.
• Collaborates with patient, caregivers, and multidisciplinary team to implement Plan of Care.
• Coordinates discharge planning and ensures completion of planning needs.
• Monitors and adjusts Plan of Care as needed with the team.
• Documents Case Management actions in the EMR and communicates with providers/payors.
• Participates in multi-disciplinary rounds to discuss discharge planning.
• Identifies system issues and proposes interventions to reduce duplications.
• Maintains knowledge of payer practices impacting the plan of care.
• Supports age-appropriate planning and communication per guidelines.
