Job summary

Lakeland Care, Inc. is seeking a part-time RN Care Manager to serve frail elders and adults with disabilities in Fond du Lac and Dodge County, with potential travel up to 2 hours within the region. You will be part of an interdisciplinary team to promote well-being and outcomes through person-centered care.

Under the Care Management Supervisor, you will assess needs, coordinate services, develop and monitor plans, support transitions, and participate in on-call rotations and in-home visits.

Qualifications

• Current Wisconsin RN license is required.
• Bachelor’s degree in nursing strongly preferred.

Responsibilities

• Conduct comprehensive assessment of member outcomes, needs and risks and reassess as conditions change.
• Coordinate acute and primary care services, care transitions and follow-up care.
• Develop, monitor and evaluate member-centered plans considering cost and effectiveness.
• Promote prevention and wellness within the care planning process.
• Implement risk mitigation strategies with the care team to support health and independence.
• Maintain member records per DHS contract and LCI policy.
• Participate in in-home visits and care conferences to address health needs.
• Join 24-hour on-call rotation as necessary.
• Participate in team meetings and internal workgroups.
