Job summary

Lakeland Care Inc. is seeking an RN Care Manager to join an interdisciplinary team serving frail elders, adults with physical or intellectual and developmental disabilities in Fond du Lac and Dodge County.

This part-time float position involves travel within the service region and in-home visits to coordinate care. As an active IDT member, you will assess needs, develop and monitor outcome-based care plans, coordinate services, and ensure DHS contract compliance while maintaining patient privacy

Qualifications

• Current Wisconsin RN license.
• Bachelor’s degree in nursing strongly preferred.
• Two+ years of skilled nursing experience preferred (gerontology/disabilities/home care).
• Ability to access members’ homes and lift up to 25 lbs.
• Valid driver’s license and adequate auto insurance.

Responsibilities

• Conduct comprehensive assessments of member outcomes, needs and risks.
• Coordinate acute and primary care services and care transitions.
• Develop, monitor and evaluate member-centered, outcome-based care plans.
• Promote prevention and wellness within the care planning process.
• Implement risk mitigation to support health, safety and independence.
• Maintain member records per DHS contract and policy; participate in in-home visits.
• Join 24-hour on-call rotation and participate in team meetings and care conferences.
• Provide nursing care in accordance with the Nurse Practice Act and HIPAA requirements.
