Job summary

Lakeland Care is seeking an RN Care Manager to join an interdisciplinary team serving frail elders and adults with disabilities in Wisconsin. You will assess needs, coordinate services, and develop person-centered plans with a focus on quality outcomes.

Responsibilities include in-home visits, care conferences, care transitions, and adherence to DHS requirements while maintaining confidentiality and HIPAA compliance. Travel to member homes is expected.

Qualifications

• Current License to practice as a Registered Nurse in Wisconsin.
• Bachelor’s degree in nursing strongly preferred.
• Two or more years of skilled nursing experience preferred, ideally in gerontology/disabilities and/or home care.
• Ability to access members’ homes which are not required to comply with ADA regulations.
• Ability to lift up to 25lbs.

Responsibilities

• Assess member outcomes, needs and risks; reassess as conditions change.
• Coordinate acute and primary care services, care transitions and follow-up.
• Develop and monitor member-centered plans, balancing costs and provider choice.
• Incorporate preventive and rehabilitative care into planning.
• Work with the IDT to implement risk mitigation for health and safety.
• Participate in ongoing training to meet DHS and policy standards.
• Maintain member records per DHS contract and policy.
• Conduct in-home visits and care conferences to address health needs.
• Participate in 24-hour on-call rotation.
