Job summary

Lakeland Care Inc. is seeking a Registered Nurse Care Manager to join an interdisciplinary team serving frail elders and adults with disabilities in Wisconsin.

This role emphasizes person-centered, outcome-based care planning and comprehensive service coordination to promote member well-being. Under the Care Management Supervisor, you will assess needs, arrange services, monitor plans, and participate in in-home visits and care conferences while ensuring DHS contract compliance and maintaining

Qualifications

• Current License to practice as a Registered Nurse in Wisconsin.
• Bachelor’s degree in nursing strongly preferred.
• Two or more years of skilled nursing experience preferred, gerontology/disabilities and/or home care.
• Ability to access members’ homes and lift up to 25lbs.
• Current driver’s license, driving record and proof of adequate insurance.

Responsibilities

• Conduct a comprehensive assessment of member outcomes, needs and risks.
• Coordinate acute and primary care services and care transitions.
• Develop, monitor, and evaluate member-centered plans.
• Promote prevention and wellness within care planning.
• Implement risk mitigation strategies in collaboration with the care team.
• Maintain and protect member records per DHS contract and policy.
• Participate in in-home visits and care conferences.
• Join 24-hour on-call rotation as needed.
• Attend team meetings and internal workgroups.
• Provide nursing care per Nurse Practice Act.
• Maintain PHI confidentiality as required by HIPAA.
