Job summary

This float position, primarily within Dodge County, involves arranging care services based on comprehensive assessments and coordinating care transitions. Applicants must have a current RN license in Wisconsin and a preferred bachelor's degree in nursing, along with two years of skilled nursing experience. Travel may be required within a 2-hour radius of the service region.

Qualifications

• Current License to practice as a Registered Nurse in Wisconsin.
• Two or more years of skilled nursing experience preferred, ideally in gerontology/disabilities and/or home care.
• Ability to lift up to 25lbs.

Responsibilities

• Provide care management and service coordination to LCI members.
• Arrange services based on comprehensive assessments.
• Monitor provision of services based on member-centered plans.
