Job summary

Lakeland Care, Inc. is seeking an RN Care Manager to join an interdisciplinary team serving frail elders and adults with disabilities in Wisconsin.

The RN Care Manager coordinates care, conducts comprehensive assessments, and develops member-centered plans in accordance with DHS contract requirements. This role emphasizes prevention, care transitions, and ongoing collaboration with the member, family, and providers.

Qualifications

• Current License to practice as a Registered Nurse in Wisconsin.
• Two (2) or more years of skilled nursing experience preferred, ideally in gerontology/disabilities and/or home care.
• Ability to access members’ homes which are not required to comply with the ADA regulations.
• Ability to lift up to 25lbs.
• Current driver’s license, acceptable driving record and proof of adequate insurance.

Responsibilities

• Conduct comprehensive assessments of members’ outcomes, needs and risks; reassess as conditions change.
• Coordinate acute and primary care services, care transitions and related follow-up care.
• Develop, monitor, and evaluate member-centered plans, considering cost and effectiveness.
• Promote prevention and wellness by incorporating preventative care into care planning.
• Implement risk mitigation strategies to promote health, safety and independence.
• Participate in ongoing training to ensure compliance with regulations and policy.
• Create and maintain member records per DHS contract and policy.
• Participate in in-home visits and care conferences with member, supports, and providers.
• Participate in 24-hour on-call rotation.
• Attend team meetings and internal workgroups.
• Provide nursing care in accordance with the Nurse Practice Act.
• Maintain confidentiality of PHI per HIPAA.
