• Valid driver’s license required (USA).

Responsibilities

• Assess individuals to identify hopes, dreams, and goals and provide supports to help achieve them.
• Support individuals in building relationships and engaging in their community.
• Assist with daily activities and community participation while safeguarding rights and promoting advocacy.
• Maintain accurate documentation including progress notes, goal tracking, and timesheets.
• Provide transportation as needed and assist with emergency procedures and safety.
