Qualifications

• High School Diploma is required.
• Experience supporting individuals with mental health or developmental needs is preferred.
• Valid driver’s license and access to reliable transportation.

Responsibilities

• Assess individuals' hopes and goals to provide supports for achievement.
• Help people build community relationships and utilize natural supports.
• Support daily living activities and participate in community life.
• Document progress, goals, and incidents; complete required logs.
• Ensure safety, rights protection, and person-centered planning.
• Provide transportation as needed and assist with emergencies and procedures.
