Qualifications

• DNP, FNP, ENP, or PA from an accredited program.
• 3+ years experience in outpatient Family Medicine/Internal Medicine/Primary Care.
• 2+ years of experience delivering virtual care.

Responsibilities

• Provide high-quality care for patients in a virtual setting.
• Use prescriptive authority as part of a collaborative practice with Physicians for comprehensive care.
• Utilize remote exams and home monitoring tools for assessments.
• Collaborate with care team including RNs, medical assistants, PCPs, behavioral health clinicians, and clinical supervisor.
• Attend meetings to support patient care (huddles, case conferencing, All Hands).
• Report to the Director, Virtual Health Assessment.
