• DNP, FNP, ENP, or PA credential from an accredited program.
• 3+ years experience in outpatient Family Medicine, Internal Medicine, or Primary Care.
• 2+ years delivering virtual care.

Responsibilities

• Provide high-quality care for patients in a virtual setting.
• Use prescriptive authority under a collaborative practice agreement with Physicians.
• Use remote home monitoring tools for virtual examinations.
• Participate in a collaborative care model with nurses, medical assistants, PCPs, and other clinicians.
• Attend required meetings to support patient care (e.g., huddles, case conferences, All Hands).
