The Columbus Organization is seeking a part-time Case Manager to coordinate person-centered Plans of Care and Service Plans for Kentucky clients. The role involves broker services, authorizations, and monthly check-ins to ensure quality care.

Remote work is available within Kentucky, with a focus on effective coordination among providers and families. Ideal candidates will have an associate degree or higher in behavioral or social sciences and at least 1 year of health or human services

Qualifications

Associate degree or higher in behavioral or social sciences.
At least 1 year of health or human services support experience.
Experience in interviewing individuals and assessing needs per program requirements.

Responsibilities

Coordinate Plans of Care and Service Plans for participants.
Broker services and set up authorizations based on Service Plans.
Monitor service delivery through monthly contact with participants and caregivers.
Conduct monthly telephonic visits with participants or as required.
Educate participants and families about resources and services.
Maintain accurate documentation of interactions and care plan updates.
