This is a remote position serving clients in Kentucky. Candidates must reside in Kentucky, preferably within or near the assigned service area, to effectively support the needs of the communities they serve.
Summary of position
The Case Manager is responsible for coordinating the person-centered Plans of Care and Service Plans for participants on-boarded by the Clinical Intake Specialist. Facilitating the selection of service providers, the Case Manager will broker services and set up authorizations based on the frequency, scope and duration of the Service Plan. The Case Manager is responsible for monitoring and confirming service delivery through monthly telephonic or in-person contact with participants and caregivers/legal guardians.
