Job summary

CareStar, Inc. is seeking a Team Case Manager for Lucas and surrounding counties to assess needs, develop care plans, and connect clients with services.

This full-time role combines remote work with occasional field visits across Ohio, emphasizing person-centered care and collaboration with clinical teams. The successful candidate will have a social work or nursing background, be licensed, and bring experience in home-based services and Medicaid navigation.

Qualifications

• Bachelor’s degree in Social Work, Counseling, Psychology, Nursing, or related field.
• Licensed Social Worker (LSW), Licensed Independent Social Worker (LISW), LPN or RN.
• Minimum 1 year of experience in home and community-based services.
• Experience with individuals with chronic conditions or mental illness.
• Strong knowledge of Medicaid, behavioral health systems, and local resources.

Responsibilities

• Conduct face-to-face eligibility evaluations and assessments.
• Maintain business-hour availability (8:00 a.m.–5:00 p.m.).
• Travel to individuals’ homes as needed, up to 2 hours each way.
• Lead care plan development and authorization of services.
• Coordinate services across behavioral health, medical, and community systems.
• Monitor services for quality and cost-effectiveness.
• Assist individuals in accessing Medicaid and essential services.
• Ensure compliance with program guidelines and confidentiality.
• Serve as primary advocate and care coordinator for participants.
