Job summary

Telos Health Systems is seeking a Psychotherapist (LMHC, LMFT, LCSW, or Licensed Psychologist – PsyD/Ph.D.) to deliver mental health evaluations and psychotherapy in Jacksonville, FL. This is a W-2, part-time (1 day per week) role with fee-for-service pay and strong administrative support.

You will join a clinician-first platform offering flexibility, autonomy, and mission-driven leadership, with care delivered across Skilled Nursing and Assisted Living facilities.

Qualifications

• Active Florida license as LMHC/LMFT/LCSW or licensed psychologist (PsyD/Ph.D.).
• Level II background check required per AHCA.
• Master’s degree for LMHC/LMFT or Doctoral degree for Psychologist.

Responsibilities

• Conduct comprehensive mental health assessments and diagnostic evaluations.
• Provide individual psychotherapy and ongoing treatment.
• Develop, implement, and update individualized treatment plans with a multidisciplinary team.
• Maintain accurate, timely, compliant clinical documentation.
• Coordinate care with facility staff and other healthcare professionals.
• Adhere to all applicable federal, state, and organizational standards and regulations.
