Job summary

Telos Health Systems is seeking a Psychotherapist (LCSW, LMHC, LMFT, or Licensed Psychologist – PsyD/Ph.D.) to provide mental health evaluations and psychotherapy in Skilled Nursing Facilities and Assisted Living Facilities. Part-time, 1 day per week, you pick the day.

Position offers clinical autonomy with strong administrative and leadership support, in a flexible hybrid model combining in-person care with virtual charting and company equipment provided.

Qualifications

• Active Florida license LMHC, LMFT, LCSW or Licensed Psychologist (PsyD/Ph.D.).
• Master’s or Doctoral degree in a mental health field.
• Ability to work independently within a clinical team.
• Experience with geriatric populations and long‑term care settings.

Responsibilities

• Conduct comprehensive mental health assessments and diagnostic evaluations.
• Provide individual psychotherapy and ongoing treatment services.
• Develop, implement, and update individualized treatment plans with a care team.
• Maintain accurate, timely, and compliant clinical documentation.
• Coordinate care with facility staff and other healthcare professionals.
• Adhere to federal, state, and organizational standards and policies.
