Job summary

Telos Health Systems is seeking a Psychotherapist ( LPCC, LMFT, or Licensed Psychologist – PsyD/Ph.D.) to provide mental health evaluations and psychotherapy services to patients in Skilled Nursing Facilities and Assisted Living Facilities. This role offers flexibility, clinical autonomy, and strong administrative and leadership support.

The position includes comprehensive benefits, a pay-per-encounter compensation model, and a sign-on bonus.

Qualifications

• Active North Carolina license as LPCC, LMFT or Psychologist.
• Ability to work independently within a clinical team.
• Strong communication and clinical documentation skills.

Responsibilities

• Conduct comprehensive mental health assessments and diagnostic evaluations.
• Provide individual psychotherapy and ongoing treatment services.
• Develop, implement, and update individualized treatment plans with a multidisciplinary team.
• Maintain accurate, timely, and compliant clinical documentation.
• Coordinate care with facility staff, medical providers, and other healthcare professionals.
• Adhere to federal, state, and organizational standards and regulations.
