Job summary

Telos Health Systems, Inc. is seeking a Psychotherapist (LMHC, LCSW, LMFT, or Licensed Psychologist – PsyD/Ph.D.) in Palatka, FL.

This part-time role will provide mental health evaluations and psychotherapy within Skilled Nursing Facilities and Assisted Living Facilities, with strong clinical and administrative support. Flexible schedule, day-time hours, and a sign-on/retention bonus of up to $5,000 are offered.

Qualifications

• Active Florida license in LMHC, LCSW, LMFT or Psychologist (PsyD/PhD).
• Master’s degree (LMHC/LMFT) or Doctoral degree (Psychologist).
• Level II background check required per AHCA.

Responsibilities

• Conduct comprehensive mental health assessments and diagnostic evaluations.
• Provide individual psychotherapy and ongoing treatment services.
• Develop, implement, and update individualized treatment plans with a multidisciplinary care team.
• Maintain accurate, timely, and compliant clinical documentation.
• Coordinate care with facility staff, medical providers, and other healthcare professionals.
• Adhere to all federal, state, and organizational standards, policies, and regulations.
