Job summary

Telos Health Systems is seeking a Psychotherapist (LCMHC, LMFT, Psychologist - PsyD/Ph.D.) to provide mental health evaluations and psychotherapy in Skilled Nursing Facilities and Assisted Living Facilities. Flexible scheduling, clinical autonomy, and strong administrative support are offered.

This role supports both part-time and full-time arrangements with full-time benefits, including comprehensive health coverage and paid time off.

Qualifications

• Master’s or Doctoral degree in a mental health field
• Active, unrestricted license in state of practice
• Ability to work independently while collaborating within a clinical team
• Strong communication and clinical documentation skills

Responsibilities

• Conduct comprehensive mental health assessments and diagnostic evaluations
• Provide individual psychotherapy and ongoing treatment services
• Develop, implement, and update individualized treatment plans in collaboration with a multidisciplinary care team
• Maintain accurate, timely, and compliant clinical documentation
• Coordinate care with facility staff, medical providers, and healthcare professionals
• Adhere to all federal, state, and organizational standards, policies, and regulations
