Job summary

Morris Heights Health Center is seeking a Part-Time PrEP Program Support Specialist to help coordinate, navigate, and grow our PrEP program. You will work with patients, clinicians, and departments to ensure access, engagement, and retention in PrEP services.

The ideal candidate is organized, comfortable with outreach, knowledgeable about HIV prevention, and able to communicate respectfully and nonjudgmentally. This non-clinical role requires HIPAA compliance and strong follow-up skills.

Qualifications

• High school diploma or equivalent.
• Strong communication, interpersonal and organizational skills.
• Ability to discuss HIV prevention and sexual health professionally.
• Maintain sensitive patient information with confidentiality.
• Manage multiple patients, referrals, and follow-up tasks.
• Basic computer skills, including Microsoft Office.

Responsibilities

• Maintain and monitor an active PrEP patient panel (~150 patients).
• Track appointments, labs, and Milestones; conduct reminders.
• Assist patients with scheduling PrEP appointments.
• Receive and track internal/external PrEP referrals.
• Follow patients from referral through consultation to initiation.
• Support outreach to individuals learning about PrEP.
• Provide PrEP, HIV prevention information and services.
• Help navigate non-clinical barriers and connect to resources.
• Maintain PrEP registry and outreach documentation.
• Track outcomes: referrals, appointments, starts, retention.
• Assist with community outreach and collaboration with teams.
• Protect confidentiality and comply with HIPAA requirements.
