Part-Time PrEP Program Support Specialist
Morris Heights Health Center is seeking a Part-Time PrEP Program Support Specialist to support the coordination, navigation, retention, and continued growth of our Pre-Exposure Prophylaxis (PrEP) program.
This position will work closely with patients, providers, HIV Services, Infectious Disease, Primary Care, Nursing, Patient Access, Population Health, and other departments to help patients successfully access and remain engaged in PrEP services.
The ideal candidate is highly organized, comfortable conducting patient outreach, knowledgeable or interested in HIV prevention and sexual health, and able to communicate with patients in a professional, respectful, and nonjudgmental manner.
This is a non-clinical patient navigation and program support position.
What You’ll Do
- Maintain and monitor the program’s active PrEP patient panel, including approximately 150 active patients.
- Track appointments, follow-up needs, laboratory monitoring requirements, and other PrEP-related milestones.
- Conduct reminder calls and outreach to patients who are overdue, have missed appointments, or have become disengaged from care.
- Assist patients with scheduling and rescheduling PrEP appointments.
- Receive and track internal and external PrEP referrals.
- Follow patients from referral through consultation and, when appropriate, PrEP initiation.
- Support outreach to individuals who may benefit from learning more about PrEP.
- Provide approved educational information about PrEP, HIV prevention, and available services.
- Help patients navigate non-clinical barriers that may interfere with accessing or continuing PrEP care.
- Connect patients with appropriate resources related to insurance, medication access, financial assistance, or community services.
- Maintain accurate PrEP registry and outreach documentation.
- Track program outcomes such as referrals, appointment completion, new PrEP starts, patient retention, and re-engagement.
- Assist with community outreach, HIV prevention activities, and initiatives designed to increase awareness and utilization of PrEP services.
- Collaborate with clinical and administrative teams throughout Morris Heights Health Center.
- Protect patient confidentiality and comply with HIPAA and other applicable confidentiality requirements.
Qualifications
- High school diploma or equivalent.
- Strong communication, interpersonal, organizational, and follow‑up skills.
- Ability to communicate comfortably and professionally about HIV prevention, sexual health, and related topics.
- Ability to maintain sensitive and confidential patient information.
- Ability to manage multiple patients, referrals, and follow‑up activities simultaneously.
- Basic computer proficiency, including Microsoft Office.
