Job summary

Comprehensive Rehab Consultant in Rutland, VT, seeks a part-time PM&R clinician (NP/APRN/PA) to deliver physiatry services in SNFs. This 16‑hour W2 role offers an 9 AM–4 PM window on weekdays, with autonomy to manage your daily workflow and collaborate with an interdisciplinary team.

You’ll work with PCPs, PT/OT/SLP colleagues to improve patient function, pursue ongoing education, and participate in culture-building activities.

Qualifications

• Nurse Practitioner/APRN/PA license required.
• Vermont license required.
• SNF or post-acute experience preferred.
• Experience in post-acute space preferred.
• Ability to work in the U.S.

Responsibilities

• Provide PM&R/Physiatry services in SNFs.
• Represent CRC with clinical leadership in facilities.
• Collaborate with PCPs to manage conditions common in SNFs.
• Work with PT/OT/SLP teams to improve function and QoL.
• Attend multidisciplinary meetings and bridge therapy and internal medicine.
