Job summary

Comprehensive Rehab Consultants is seeking a part-time PM&R Clinician (NP/PA) to provide rehabilitation services at a designated SNF in Lebanon, VA. The role emphasizes autonomous clinical leadership, collaboration with therapy teams, and comprehensive patient care in a post-acute setting.

The ideal candidate holds NP/PA licensure in Virginia with post-acute or SNF experience, and flexibility around a daytime schedule.

Qualifications

• Nurse Practitioner/APRN/PA license required.
• Virginia license required.
• Must be able to work in the U.S.
• Post-acute/SNF experience preferred.

Responsibilities

• Provide specialized PM&R/Physiatry services as a rehab consultant.
• Represent CRC in your facility with clinical leadership and collaboration with the care team.
• Collaborate with PCPs to manage conditions common in SNFs (e.g., CVA, progressive neuro conditions, postoperative ortho).
• Work with PT/OT/SLP teams to improve function and quality of life.
• Attend multidisciplinary meetings and bridge therapy with internal medicine.
