Job summary

Comprehensive Rehab Consultants in Amherst, VA is seeking a part-time PM&R clinician to deliver rehabilitative medicine at designated SNFs. You will collaborate with an interdisciplinary team, guide post-acute care plans, and promote patient independence.

Ideal candidates are licensed NPs/APRNs/PAs with Virginia licensure, experienced in post-acute settings, and capable of autonomous practice in a SNF environment. Flexible weekday hours support work-life balance.

Qualifications

Responsibilities

• Provide PM& R/Physiatry services as a rehab consultant.
• Represent CRC in your facility with strong clinical leadership.
• Collaborate with PCP teams for SNF care plans.
• Work with PT/OT/SLP to improve patient function and quality of life.
• Follow therapy patients to track progress.
• Assess and manage barriers to therapy (pain, spasticity, neurogenic issues).
• Bridge the gap between therapy and internal medicine during multidisciplinary meetings.
• Engage in culture-building and ongoing didactic education.
