Job summary

Fortis Care is seeking a Palliative Care Advanced Practice Provider (NP or PA) to serve as the primary palliative clinician in a Savannah skilled nursing facility. The role emphasizes symptom management, serious-illness communication, and patient-centered care for geriatric residents.

Qualifications

• Licensed NP or PA in Georgia.
• 1 year of palliative care experience preferred (not hospice).
• Hospice/palliative care certification preferred or willingness to obtain within 12 months.

Responsibilities

• Provide palliative care consultations and follow-up, including symptom management and treatment planning.
• Lead goals-of-care and advance care planning discussions with patients and families; document appropriately.
• Collaborate with SNF care team to develop individualized care plans.
• Support care transitions and medication reconciliation.
• Maintain compliant documentation of assessments and plans.
• Participate in quality initiatives to reduce acute-care utilization.
