Job summary

Cornerstone Clinical Partners in Henderson, NC seeks a part-time Primary Care Nurse Practitioner/Physician’s Assistant (NP/PA) to provide on-site care to residents in long-term care facilities. This is a part-time position ideal for building meaningful patient relationships within a collaborative care environment.

The role offers a flexible 3-day-per-week schedule, with an emphasis on high-quality patient care and teamwork among clinicians and facility staff.

Qualifications

• Active and unrestricted Physician Assistant license in North Carolina.
• 6+ months of experience in long-term care or skilled nursing facilities.
• Excellent communication, clinical, and critical-thinking skills.
• Ability to lift up to 20 pounds and stand/walk 6–8 hours per day.

Responsibilities

• Provide primary care services to residents in skilled nursing and long-term care facilities.
• Perform physical exams and assess medical history.
• Diagnose and treat chronic and acute conditions.
• Order and interpret diagnostic tests and lab results.
• Develop and adjust individualized care plans and monitor treatments.
• Prescribe medications and evaluate treatment effectiveness.
• Collaborate with facility staff and interdisciplinary teams.
• Educate patients and families about treatment options and care goals.
• Maintain a safe, respectful environment for residents.
