Job summary

HealthPartners is seeking a Certified Adult/Geriatric or Family Nurse Practitioner or Physician Assistant to join our Palliative Care team in Minnesota. The APP will provide direct patient care and consultative services, collaborating closely with physicians and other healthcare professionals to deliver state-of-the-art palliative care.

The role includes comprehensive history taking, physical and psychological assessments, mentoring staff, and contributing to an interdisciplinary care plan.

Qualifications

• Completion of a physician assistant or nurse practitioner program.
• Certification via appropriate discipline.
• MN Licensure.
• Current BLS, ACLS certification.
• DEA registration.

Responsibilities

• Collaborate with physicians and healthcare staff to deliver Palliative Care.
• Perform comprehensive histories, chart reviews, and physical/psychological assessments.
• Provide direct patient care and consultative services within an interdisciplinary team.
• Mentor, coach and support staff while managing a patient caseload.
• Contribute to education of other healthcare professionals in patient care aspects.
