Job summary

AuthoraCare Collective is seeking a Nurse Practitioner for Palliative Care in our facility-based program. This full-time role supports Forsyth, Davie and Davidson Counties with a schedule of Monday through Friday, 8:00am–5:00pm, in a collaborative, multi-disciplinary environment.

The NP will provide consultative palliative care to adults with serious illness in skilled nursing, memory care, and other residential settings.

Qualifications

• Graduate of an accredited School of Nursing with completion of an accredited NP program.
• Current, unrestricted NP license and RN license in North Carolina, with prescriptive authority as required for the role.
• Current national board certification as an NP in an applicable population focus.
• Current DEA registration and controlled-substance prescribing authority, if required by assigned duties.
• Minimum of 2 years of clinical experience in palliative care, hospice, geriatrics, LTC, SNF, acute care, or outpatient practice required.
• Palliative care or hospice experience strongly preferred; specialty certification in hospice and palliative nursing encouraged.
• Excellent clinical assessment, communication, relationship-building, and serious-illness communication skills.
• Other: Valid state-issued driver's license required. Must carry automobile liability insurance at limits required by agency. Must have own transportation.

Responsibilities

• Conduct comprehensive palliative assessments and develop individualized plans of care.
• Evaluate and manage pain and distressing symptoms.
• Prescribe medications or provide recommendations within scope of practice.
• Coordinate care with facility staff, physicians, and families.
• Document clearly and maintain regulatory compliance in EMR.
• Participate in on-call coverage as assigned.
• Ensure timely hospice eligibility evaluation when appropriate.
• Provide follow-up visits based on clinical need and program criteria.
