Job summary

• Board certification for NP or PA and active, unrestricted license in the operating state.
• Master's degree in Nursing (NP) or PA program.
• Experience in skilled nursing or long-term care preferred.
• Ability to work independently in an autonomous role.

Responsibilities

• See 16–20 patients per day, with a mix of chronic care and acute issues.
• Conduct annual wellness visits, H&Ps, and review meds and orders.
• Coordinate with collaborating physicians, specialists, and facility staff to ensure seamless care.
• Engage with families and participate in care conferences as appropriate.
