Northeast Healthcare Recruitment, Inc. is seeking a Nurse Practitioner or Physician Assistant for a part-time role in a skilled nursing facility.

You will manage your own panel, provide primary and episodic care, and build long-term relationships with residents and families. The role offers flexible scheduling, 6-hour morning shifts, charting from home, and productivity-based compensation with potential earnings up to $624/day for high-volume teams.

Qualifications

• Board certification required (e.g., AGNP, FNP for Nurse Practitioners; PA-C for Physician Assistants) — active and unrestricted.
• Valid, unrestricted NP or PA license in the state of operation.
• Master's degree in Nursing (NP) or from an accredited PA program — required for the credential.
• Long-term care or skilled nursing experience strongly preferred.
• Strong independent clinical judgment and the ability to move quickly in a fast-paced setting.

Responsibilities

• See roughly 16–20 patients per day, mix of long-term chronic care and acute issues.
• Perform annual wellness visits and H&Ps, manage chronic and co-morbid conditions.
• Respond to acute changes and review meds and orders.
• Coordinate with collaborating physicians, specialists, and facility staff to keep care seamless.
• Stay in touch with families and participate in care conferences when appropriate.
