Northeast Healthcare Recruitment, Inc. is seeking an experienced Nurse Practitioner or Physician Assistant to deliver primary and episodic care in a skilled nursing facility.

This full-time role supports independent practice with a dedicated panel and ongoing relationships with residents and families. You will see 16-20 patients daily, conduct annual wellness visits and H&Ps, review meds, coordinate with physicians and facility staff, and document at the point of care using an integrated EHR

Qualifications

• Board certification required (AGNP/FNP or PA-C) with active, unrestricted license.
• Valid, unrestricted Nurse Practitioner or Physician Assistant license in Michigan.
• Master's degree in Nursing or PA program from an accredited school.

Responsibilities

• See 16–20 patients daily, including wellness visits and acute care.
• Review meds, orders, and coordinate with physicians and facilities.
• Document using integrated EHR and communicate with families.
