Job summary

Northeast Healthcare Recruitment, Inc. (NEHCR) presents a full‑time Nurse Practitioner or Physician Assistant role in a skilled nursing facility. You will manage your own patient panel, deliver primary and episodic care, and build long‑term relationships with residents and families.

Qualifications

• Board certification required, active and unrestricted.
• Unrestricted state licensure as NP or PA.
• Master's degree in nursing (NP) or PA program required for credentialing.
• Skilled nursing or long-term care experience strongly preferred.
• Strong independent clinical judgment and ability to move quickly in a fast-paced setting.

Responsibilities

• See roughly 16–20 patients per day, including chronic and acute issues.
• Perform annual wellness visits and H&Ps; manage chronic conditions.
• Coordinate with physicians, specialists, and facility staff to ensure seamless care.
• Communicate with families and participate in care conferences as appropriate.
