This independent role emphasizes continuity of care, with you managing your own panel and building long-term relationships with patients and families. You will see roughly 16–20 patients per day, with 6-hour rounds, chart from home, and a productivity-based pay model.

Qualifications

• Board-certified NP or PA with active, unrestricted license.
• Master’s degree required for credential.
• Skilled in long-term care experience preferred.
• Strong independent clinical judgment in fast-paced setting.

Responsibilities

• See roughly 16–20 patients daily, including wellness visits and acute issues.
• Coordinate with families, collaborating physicians, and facility staff to ensure seamless care.
• Document using integrated EHR and participate in care conferences when appropriate.
