Responsibilities

• Coordinate care with interdisciplinary team to promote health and wellness.
• Communicate relevant patient data to the care team promptly.
• Adhere to infection prevention standards and wound care procedures.
• Assist with activities of daily living and vital signs documentation.
• Support patient and family education and ensure room readiness.
