Powerback Rehabilitation is seeking a Physical Therapist for in-home outpatient services in Concord, NH. This part-time, mobile role focuses on elder populations, delivering customized therapy in patients' homes or living facilities.

The PT will evaluate and treat patients, develop care plans, train caregivers, and collaborate with the rehab team to optimize outcomes. A valid US PT license is required; a flexible schedule is available.

Competenze

• Must hold a valid Physical Therapy license in the state of practice.
• Master's degree in Physical Therapy or equivalent.
• Bachelor's deg. in Physical Therapy plus 5 years PT experience also acceptable.

Mansioni

• Deliver outpatient rehabilitation services in the home setting for older adults.
• Assess needs, develop and implement PT plans of care and document care.
• Coordinate referrals to other disciplines as needed.
• Educate patients and caregivers to promote independence and safety.
• Supervise PTAs, students, and support staff as allowed by state licensure.
• Ensure safety, maintain equipment, and follow scheduling and billing requirements.
