Job summary

Powerback Rehabilitation is seeking a licensed Physical Therapist to deliver mobile outpatient therapy in Chelmsford, MA. The role covers in-home visits or in independent living facilities, with a flexible schedule and patient-centered care.

The therapist will assess needs, design, implement, and adjust therapy plans, educate patients and caregivers, and collaborate with other rehab team members to promote independence and functional improvement.

Qualifications

• Must have a valid Physical Therapy license in the state(s) of practice.
• Master's degree in Physical Therapy or equivalent.
• Bachelor's degree in Physical Therapy plus 5 years of PT experience (if applicable).

Responsibilities

• Provide in-home outpatient rehabilitation services to older adults.
• Develop and deliver individualized PT programs for home environments.
• Supervise assistants and students per licensure requirements.
• Collaborate with rehabilitation team to address comprehensive needs.
