Qualifications

• Graduation from an Accredited School of Nursing with a BSN preferred.
• Must be in good standing with the State Board of Nursing.
• 3 years of supervisory experience preferred.
• Long Term Care experience preferred.
• Proficient in Microsoft Suite; PCC knowledge preferred.

Responsibilities

• Oversee assessments to align with guidelines and PDPM processes.
• Lead weekly PDPM meetings for skilled patients.
• Track QM and QASP performance and provide data-driven recommendations.
• Provide PDPM and MDS training to new coordinators.
• Coordinate MDS data entry and electronic submissions.
• Coordinate interdisciplinary participation in MDS and care planning.
• Schedule and conduct resident care conferences and MDS reviews.
