Job summary

Brown Health & Rehab in Royston, GA is seeking an LPN RAI Coordinator for a part-time, day shift. The role supports assessment and care planning, coordinates with nursing staff and therapists, and ensures compliance with MDS and regulatory standards.

Starting pay ranges from $32.40 to $35.57 per hour with weekly pay and shift differentials. Qualifications include an active Georgia LPN license and a nursing degree or approved LPN program; strong computer skills (Outlook, Excel, Word, PowerPoint)

Qualifications

• Nursing Degree or approved LPN program is required.
• Active LPN license to practice in Georgia is required.
• Strong computer skills including Outlook/Excel/Word are expected.

Responsibilities

• Facilitates communication between nursing center departments and patients/families for effective assessment/care plans.
• Establishes open lines of communication with consultants and interdisciplinary team members.
• Assists Director with patient/family interviews and evaluation of medical data to determine needs.
