Qualifications

• Nursing Degree from an accredited college or university or is a graduate of an approved LPN program.
• An active LPN License to practice in the State of Georgia.
• Proficiency in Microsoft Office applications and effective communication.

Responsibilities

• Facilitates communication between nursing center departments and patient/family members to provide an effective assessment/care plan process.
• Establishes open lines of communication with consultants.
• Assists Director with patient or family interviews for assessment purposes.
• Assists Director with evaluation of each patient’s condition and medical data.
• Coordinates schedule with nursing center staff and others to provide for participation.
• Promotes the image and reputation of the System by servant leadership and open communication.
