Job summary

Oxley Park Health & Rehab in Lyons, GA is seeking a part-time LPN to join our compassionate nursing team. You will support assessment and care planning, coordinate with interdisciplinary staff, and help ensure care plans align with patient wishes.

The role emphasizes strong communication, detailed documentation, and adherence to regulatory standards. Starting pay ranges from $27.75 to $31 per hour with evenings, nights, and weekend shift differentials; weekly pay and benefits are included.

Qualifications

Nursing degree from an accredited college or university or graduate of an approved LPN program.
Active LPN license to practice in Georgia.
Ability to read and interpret safety rules, operating and maintenance instructions and procedure manuals.

Responsibilities

Facilitates communication between nursing center departments and patient/family members to provide an effective assessment/care plan process.
Assists Director with patient or family interviews for assessment purposes.
Assists Director with evaluation of each patient’s condition and medical data to determine special needs, or the need to amend the admission assessment.
Helps to coordinate development of a care plan for each patient with problems/needs, interventions and measurable goals.
Monitors the MDS process for accuracy according to MDS guidelines and standards.
