Qualifications

• Nursing degree from an accredited program or completion of an approved LPN program.
• Active LPN license to practice in Georgia.
• Proficiency with Microsoft Office applications (Outlook, Excel, Word, PowerPoint).
• Strong organizational and prioritization skills.
• Effective verbal and written communication with patients and families.

Responsibilities

• Facilitates communication between nursing center departments and patient/family to support care plans.
• Establishes open lines of communication with consultants and interdisciplinary team members.
• Assists Director with patient interviews and assessment-related activities.
• Participates in developing preliminary and comprehensive care plans for patients.
• Coordinates scheduling and participation in care plan meetings with staff and family.
• Ensures documentation accuracy, timeliness, and proper signatures in care plans.
