Starting pay ranges from $30.49 to $33.40 per hour with shift differentials in evenings, nights and weekends, plus weekly pay and added management pay.

Qualifikationen

• Nursing degree from an accredited college or university or a graduate of an approved LPN program.
• Active LPN License to practice in the State of Georgia.

Aufgaben

• Facilitates communication between nursing center departments and patient/family members to provide an effective assessment/care plan process.
• Establishes open lines of communication with consultants and interdisciplinary team members to deliver comprehensive patient care.
• Assists Director with patient or family interviews for assessment and care plan development.
• Coordinates the development of preliminary and comprehensive care plans for each patient with measurable goals.
• Ensures care plans and assessments are completed in a timely manner and signed with proper dates and signatures.
• Monitors the MDS process for accuracy according to guidelines and supports team education on MDS procedures.
