Job summary

Four County Health & Rehab in Georgia is seeking a Part-Time LPN with an active GA license to join our mission-driven team. Starting pay ranges from $27.75 to $31 per hour with evenings, nights and weekend differential, and weekly pay.

The role focuses on coordinating patient care plans, interfacing with multiple departments, and education of staff to ensure timely, compliant care. Competitive benefits include PTO, holidays, 401(k) with match, and more.

Qualifications

• Minimum Qualifications include Nursing Degree or graduation from an approved LPN program.
• Active LPN License to practice in the State of Georgia.

Responsibilities

• Facilitates communication between nursing center departments and patient/family members to support care plans.
• Establishes open lines of communication with consultants and interdisciplinary team members.
• Assists Director with patient/family interviews and evaluation of patient conditions for admission and care planning.
• Coordinates development of preliminary and comprehensive care plans with measurable goals.
• Ensures care plans are completed timely and signed where required.
• Educates associates about patient care plans and reviews plans prior to administering care.
