Qualifications

• Nursing Degree from an accredited college or university or a graduate of an approved LPN program.
• Active LPN License to practice in the State of Georgia.
• Proficient in computer applications and MS Office suite.

Responsibilities

• Facilitates communication between nursing center departments and patients/families to support care plans.
• Maintains open lines of communication with the interdisciplinary team to ensure a holistic patient plan.
• Assists Director with evaluating patient data to determine needs and amend admissions assessments.
• Coordinates assessment and care plan meetings to ensure timely, compliant documentation.
