The LPN will provide direct nursing care, monitor residents’ health, administer medications, and develop individualized care plans to support six residents in a compassionate, respectful environment. Qualifications

• CPR certification is required.
• PA driver's license is required.
• Experience with individuals with intellectual and developmental disabilities is preferred.
• Ability to lift 25–50 lbs and assist with transfers.
• Strong communication and teamwork skills are essential.

Responsibilities

• Provide direct nursing care and health monitoring for residents.
• Administer medications accurately and monitor for side effects.
• Develop, implement, and evaluate individualized care plans.
• Maintain medical records and ensure compliance with ICF/ID regulations.
• Assist residents with activities of daily living (hygiene, dressing, feeding).
• Accompany residents to medical and dental appointments.
• Respond to health emergencies and implement appropriate interventions.
• Collaborate with the interdisciplinary team and participate in care planning.
• Maintain adequate medical supplies and equipment.
• Support residents in community activities and outings.
