St. Joseph’s Center is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our nursing team in a community-based Intermediate Care Facility home. What You’ll Do

• Provide direct nursing care and health monitoring for residents.
• Administer medications accurately and monitor for side effects.
• Develop, implement, and evaluate individualized care plans.
• Maintain medical records and ensure compliance with ICF/ID regulations.
• Assist residents with activities of daily living (hygiene, dressing, feeding).
• Accompany residents to medical and dental appointments.
• Respond to health emergencies and implement appropriate interventions.
• Collaborate with the interdisciplinary team and participate in care planning.
• Maintain adequate medical supplies and equipment.
• Support residents in community activities and outings.
