Job summary

OneWell Health Care in Pennsylvania is seeking a Licensed Practical Nurse (LPN) to provide skilled nursing in home and community settings for individuals with IDD. You will work under the supervision of an RN Case Manager, implement care plans, administer medications, and support families to maintain health and quality of life.

This field-based role requires PA licensure, CPR/First Aid, and a minimum of one year of nursing experience, with preference for IDD/home health experience.

Qualifications

• Completion of an accredited Practical Nursing program approved by the Pennsylvania State Board of Nursing.
• Current and active LPN license in Pennsylvania with CPR and First Aid certification.
• Minimum of one year nursing experience; IDD/home health experience preferred.

Responsibilities

• Implement individualized care plans and administer medications and treatments.
• Perform clinical procedures including wound care, catheter care, feeding tube management, and vital sign monitoring.
• Educate families and train new nursing staff; participate in interdisciplinary meetings.
• Maintain infection control, safety practices and accurate documentation.
